1040 - checklist
This is a list of the most common items that we’ll need to prepare your return.
- Social Security Cards & Birth Dates for anyone you are claiming as a dependent
- State ID for taxpayer and spouse
- W-2’s / 1099’s from Interest, Dividends, Retirement, Social Security, Disability, Unemployment, Gambling Winnings, Businesses
- Amount of IN Excise tax on License Plates , Safe Deposit Box fees
- All contributions to regular or Roth IRA’s; any information concerning rollovers or Roth Conversions
- Year-end statements of Mortgage Interest (Form 1098), and/or Home Equity Loans, lines of credit. Provide copies or original receipts for Real Estate Taxes Paid if not on the Form 1098. For Michigan please bring your taxable value of your homestead
- Copies of all LLC, Partnership or S-Corporation K-1’s
- Legal documents for the sale, closing or purchase of a business during the year
- If you bought, sold or refinanced Real Estate, then the HUD Statement from your closing for each transaction
- Day care expenses, name or provider, address, Federal ID number or Social Security Number and amount paid
- If you sold any shares of any stocks or mutual funds you should receive a 1099B
- Education expenses, college name, year in school, form stating tuition and fees paid (form 1098-T), Student Loan Interest paid – (must be in the taxpayer or spouse name
- If you are claiming auto mileage as a deduction for business, rental properties or unreimbursed employee expenses, we need to know: Total miles, commuting miles, and business miles driven for the year
- If you lease your car or are deducting actual expenses, please provide: original value of the car, date of lease, all expenses for the lease payments, gas, car washes, licenses, insurance, tires, repairs, etc.
- List of medical expenses, separated by Doctor, Dentist, Hospital, Prescriptions, Health Insurance Premiums, Long Term care premiums, mileage that relates to all medical expenses, insurance form 1095-A, B, or C (if applicable)
- If you rent – your Landlords Name, Address and the amount of rent paid for the year
- All Legal Documents for a DIVORCE DECREE, - Legal Child Custody Papers to claim your child as a dependent
- Voided check for Checking or Savings account when you wish to have your refund direct deposited to your account.
- Copies of Last year’s Federal & State Returns. Copies of any Federal, State or Local correspondence during the year.
- Any Depreciation Schedules for your business equipment and/or buildings
- If you moved in 2017 – we will need the address where you lived as of Jan 1, 2017 and your present address
- If you lived out of state and moved into Indiana we will have to know the dates (from & to) you lived in what state
